The mission of the Peter Emily International Veterinary Dental Foundation (PEIVDF) is to provide advanced veterinary dental services for the benefit of captive animals located in facilities in the US and abroad as well as increasing the availability of veterinary dental education.

- Donor Type (either personal or business)
- Donor address (for tax receipt mailing)
